The Case Management Coordinator is responsible for compliance with CMS Conditions of Participation regarding Utilization Review and Discharge Planning. The Case Management Coordinator is responsible for the coordinating the provisions of care regarding discharge planning and patient needs for care, treatment, and services after discharge or transfer are met. The Case Management Coordinator will integrate the functions of utilization review, discharge planning and resource management to ensure the receipt of high quality, cost efficient medical outcomes.
Required Education: Activities Coordinator certification or willing to obtain within six months of hire.
Preferred Education: Current California RN License, BSN and/or MSW, and/or Public Health Certification.
Experience: A minimum of 3 years’ experience in case management or utilization management, or 5 years’ experience as a registered nurse in an acute hospital setting. Experience in case management or quality assurance preferred.
Knowledge, Skills and Abilities: Ability to work independently of others, be self-directed, have good verbal and written communication skills, and have the ability to deal objectively with a wide variety of people and situations.
